For cortisol, early morning is the reference point. This leads to a simple consequence. A cortisol value without a time cannot be interpreted because the reference range on the report applies to a specific time of day. Why the time of day makes all the difference for cortisol

For most blood values, the time of day is a minor detail. For cortisol, it is the main issue, and the reason is stated in the reference ranges themselves.

The MSD Manual, Professional Edition, gives a range of 5 to 25 micrograms per deciliter for early morning between 6 and 8 a.m., equivalent to 138 to 690 nanomoles per liter. By midnight, the level gradually drops to below 1.8 micrograms per deciliter, or below 50 nanomoles per liter (as of December 2024).

The same source draws the practical conclusion and notes that the time of day when blood is drawn matters (as of April 2, 2025). For cortisol, this is not a minor detail but the prerequisite for making the result interpretable at all.

What the reference range on the report presupposes

A reference range is not a boundary between health and illness, but a statistical measure. IQWiG explains how it is established: The limits are set so that 95 percent fall within the range, while 5 percent of people fall outside it even though they are healthy (as of 02/04/2025).

These 95 percent were determined under specific conditions, and for cortisol, the time of day is one of them. Measuring at 2 p.m. and comparing the result with a morning range means comparing it with a standard that was never intended for that time.

The same source adds a second limitation: reference ranges may vary from one laboratory to another, and the range shown on your own report is always the relevant one (as of 02/04/2025). A value found online is therefore not suitable for comparison.

What matters the evening before

Preparation does not begin on the morning of collection, but the evening before. The reason is sleep, and with this value, sleep is not a peripheral issue.

Cortisol follows the sleep–wake cycle. Its lowest point is around midnight, and its highest point is in the early morning after waking. A sleepless or significantly shortened night shifts this rhythm, and therefore also shifts the point captured by the sample.

In practice, this means the best time for collection is a morning after an ordinary night. Not after a night shift, not after a flight across several time zones, and not after four hours of sleep.

If this ordinary morning is not possible at the moment, postponing is the better decision than measuring. A kit does not expire after a certain number of days, whereas an unusual night makes the result useless for comparison.

Why sleep carries more weight here than with other values

A short night makes little difference to a cholesterol level. With cortisol, sleep sets the rhythm of the curve itself, which is why it comes first in this process.

There is also a second reason that goes beyond the measurement itself. Sleep deprivation and inadequate recovery rank high among the causes of persistent fatigue (MSD Manual, professional edition, as of April 2025). Anyone measuring because of exhaustion who has also been sleeping too little for weeks already has part of the answer before the sample is taken.

This does not argue against the measurement. It argues for noting the previous night’s sleep duration as well, because it is part of interpreting the result.

Collection in five steps

The following sequence describes a morning when the sample is collected at home. The order is not arbitrary, because each step prepares for the next.

The evening before: unpack the kit and set the appointment

Lay out all the components and read the enclosed instructions all the way through once. Set the alarm so there is enough time between getting up and collecting the sample. Anyone who searches for things in the morning unintentionally shifts the time later.

Get up and settle in calmly

After waking up, remain at rest for a short while. No exercise, cold shower, or rushing. Physical exertion is one of the stimuli this hormone responds to, and it pushes the result upward.

Collect the sample in the early-morning window

The MSD Manual refers to the morning as the period between 6 and 8 a.m. (as of December 2024). Anyone who follows this window is measuring against the standard for which it was designed. The specific steps are provided in the instructions for the relevant kit.

Record the time and circumstances

Date, time to the minute, duration of the previous night’s sleep, medications, and anything unusual. These four lines will later determine whether two results can be compared.

Send it on its way the same day

The sample should be sent back without delay. The shorter the time between collection and arrival at the laboratory, the fewer questions arise later about the sample’s stability.

Step four is the one most often skipped, and also the only one that costs nothing. Without a recorded time, a result is just a number without a reference point.

What else can shift the result

The time of day is the biggest, but not the only, influencing factor. Three others belong on the same list as the time.

Medications

Glucocorticoid medications are the clearest case. Prolonged use can cause excess cortisol, directly affecting what is being measured (MSD Manual, Professional Edition, as of December 2024).

That is why every preparation should be written down, including ointments and sprays, even if they were used only temporarily. The decision about which of them matters belongs in the doctor’s office, not at the kitchen table.

Acute stress on the morning of the sample collection

A missed train, a heated argument, a painful needle prick on the second attempt: Anything that puts the body on alert can push the result upward. This is not a disruption of the procedure, but precisely the response this hormone is responsible for.

In practical terms, this means that an unsettled morning is a bad morning for testing. Postponing it is the better choice here, too.

Special circumstances

Conditions associated with elevated cortisol levels include obesity, depression, and polycystic ovary syndrome, among others (MSD Manual, professional edition, as of December 2024). Anyone affected by these conditions should take them into account when interpreting the result and mention them during the discussion.

This is not a reason to forgo testing. It is a reason not to interpret the result on its own.

Morning, late morning, and evening compared

Three time windows occur in practice. The table compares them using the same four criteria.

The middle column is the reason for this article. It describes the situation that occurs most often in everyday life and causes the most unnecessary worry.

Why the second measurement needs to be taken at the same time

A single cortisol value describes one morning. It becomes meaningful only when a second value is available for comparison, and that comparison has one condition.


A value without a recorded time is a value without a reference point.

If the first sample was collected at 7 a.m. and the second at 10 a.m., you are measuring the difference between two times of day, not the change over half a year. The difference between these two interpretations is substantial.

The same time is therefore the most important rule for repetition, closely followed by the same circumstances: an ordinary night, no exertion beforehand, and the same type of weekday. Anyone who measures on a Sunday for the first appointment and on a Monday before a presentation for the second has compared two mornings that have little in common.

Which interval is appropriate

We found no generally applicable information on cortisol for this point in the sources we reviewed. What can be said follows from the question you want to answer.

If the aim is to assess changes after restructuring your daily routine, an interval of several months makes sense because habits are not reflected over just a few weeks. If the aim is to interpret a single abnormal value, the decision about repeating the test belongs in the medical practice anyway.

If the sample is taken at a medical practice

The difference between having a sample taken at home and having one taken at a medical practice lies less in the blood than in the calendar. At a practice, the available appointment determines the time; at home, you determine it.

That is the one genuine advantage of self-testing for this value. An 11 a.m. appointment makes little difference for many values, but for cortisol it is a limitation that is rarely discussed.

At the same time, the medical practice has an advantage that no kit can offer. The value can be interpreted immediately there, further procedures can be carried out there, and someone familiar with the accompanying signs is available.

If you want to combine both, do this: Ask for an early appointment if cortisol is being tested, and mention the time of your previous measurement, if you have one.

What to bring to the medical practice

A result from a self-test is useful in a consultation if it is complete. Complete means the results sheet with the reference range from the laboratory that analyzed it, along with your note of the date and time.

The reference range is therefore essential because it can vary between laboratories, and the range on your own report is always the authoritative one (IQWiG, as of April 2, 2025). Without it, there is a number that no one can interpret.

What you can leave out, however, is your own interpretation. A preliminary assessment narrows the conversation, and the values speak for themselves once the time and circumstances are noted alongside them.

Who benefits from the measurement

A morning measurement at a specific time is useful for some questions and not for others. The comparison makes this clear in both directions.

Useful for you if …

You want to establish a baseline value with the date and time, against which something can be compared in six months.

You can arrange an early morning so that the sample is sent back the same day.

Cortisol is one value among several for you, rather than the single number on which a decision depends.

You want to bring numbers to a consultation instead of collecting them for the first time at the initial appointment.

Probably not if …

There is suspicion of excess cortisol. Other methods are intended for this and belong in a medical practice.

You cannot manage the early morning. A measurement around midday cannot meaningfully be compared with the morning range.

You work shifts or have just crossed several time zones. In that case, the rhythm itself is in question.

You hope a single number will explain your exhaustion. A morning measurement cannot provide that information.

What a capillary blood test can show here

A capillary blood self-test provides a single time-point measurement. Its advantage for cortisol is that you choose the time yourself; its disadvantage is that it remains limited to that time point.

Chapter at a glance

A capillary blood self-test provides cortisol as a single time-point value. Its advantage over an appointment at a medical practice is that you can choose the early morning yourself. The value does not represent a daily profile and is not intended to investigate cortisol excess. Limitations: what the time does not resolve

The first limitation is the most important. Choosing the correct time does not turn a single time-point value into a daily profile. It ensures that the number matches the benchmark, and nothing more.

The second limitation concerns the diagnostic workup. For suspected cortisol excess, midnight salivary cortisol, free cortisol in 24-hour urine, and the dexamethasone suppression test are recommended (MSD Manual, professional edition, as of December 2024). None of these procedures can be replaced by a morning value.

The third limitation is a gap in the sources. We found no reliable information on how far a value may differ at the same time on two days without anything having changed. That is why the text includes no number on this.

The fourth limitation is the reference range itself. Five percent of healthy people fall outside it without anything being wrong (IQWiG, as of April 2, 2025). A value just beyond the limit is a reason for a discussion, not a diagnosis.

It is easy to object that an article about the right time leaves little behind if the time ultimately solves so little. The benefit nevertheless remains clear: It is the prerequisite for everything else to have a basis at all. Without it, every interpretation begins with an unknown that can no longer be added later.

Why the collection time matters

If you take away one action from this article, make it this: Write down the time of your sample collection to the minute and place the note with the report.

The reason is unremarkable and lasts a long time. This one line determines whether your next measurement becomes a comparison or just a second isolated number.

The initial question was which time matters. The answer is: early morning, because the standard is designed for that time. More important than the answer is making a habit of recording it at all.

If you miss the morning, it is better to reschedule the test for another day than to compare the result against an unsuitable standard.

Is a single cortisol value enough?

Not for a medical evaluation. To investigate excess cortisol, midnight salivary cortisol, free cortisol in 24-hour urine, and the dexamethasone suppression test are used (MSD Manual, as of December 2024). However, a single morning value can be useful as a baseline for later comparison, provided that the time and conditions are recorded.

How do I compare two cortisol values correctly?

The same time, comparable conditions, and the same reference range. Because reference ranges can vary from one laboratory to another, and the range on your own report always applies (IQWiG, as of April 2, 2025), two values from the same laboratory are easier to compare than two from different laboratories. For each value, note the date, time, amount of sleep, and medications.
