What Cystatin C is and where it comes from

Cystatin C is a small protein molecule. It inhibits certain protein-degrading enzymes in the body, and this function initially has nothing to do with the kidneys. What matters for laboratory diagnostics is something else: where it is produced and where it goes.

The MSD Manual, professional edition, describes both in a single sentence. Cystatin C is produced by all nucleated cells and filtered by the kidneys, which is why it can be used to assess kidney function (as of 2025).

Production therefore continues evenly throughout the body, regardless of how much muscle a person has. If the kidneys’ filtration capacity declines, the protein accumulates in the blood. The value rises.

Why a lab value can reveal something about an organ

The kidneys cannot be observed working directly. Instead, a proxy is measured: a substance that the body continuously produces and that almost exclusively disappears through the kidneys.

If the concentration of such a substance in the blood rises, there are two possible explanations. Either more of it is produced, or less of it is excreted. A useful kidney value is therefore one for which the first explanation is as unlikely as possible.

Why a second kidney value is needed at all

The standard value for kidney function is called creatinine. The Institute for Quality and Efficiency in Health Care describes it as a breakdown product of muscle that is excreted through the kidneys (as of March 20, 2025). The weakness lies precisely in this half-sentence.

People who have little muscle mass or barely use it produce less creatinine. The blood level remains low, and the calculation used to estimate filtration performance from it turns out too favorable. Nephrologist Jan Galle summed this up in a sentence in Deutsches Ärzteblatt that conclusively clarifies the issue for the affected groups.

The affected groups include more people than the sentence might suggest: older people with age-related muscle loss, people recovering from prolonged illness, very slim women, and people with chronic diseases that cause muscle loss.

Cystatin C does not have this problem. According to the MSD Manual, its plasma concentration is independent of sex, age, and body weight (as of 2025). This eliminates creatinine’s biggest confounding factor.

Why the kidneys remain silent for so long

There is a second reason why a sensitive value is needed at all. The kidneys make themselves known late. The Institute for Quality and Efficiency in Health Care describes the early stages of chronic kidney disease as stages in which urine tests initially show signs of damage while kidney function is still normal or only mildly impaired (as of August 14, 2024).

An organ that causes symptoms only late will become noticeable through test values—or not at all. This is not a call for concern, but a description of how the roles are divided: here, symptoms are the poorer warning system, while numbers are the better one.

And the later an organ makes itself known, the more depends on the sensitivity of the value used to observe it. Galle calls cystatin C an early-rising marker of impaired kidney function (2016), and that is the real reason for the second value.

The second weakness lies in the measurement method itself

In addition to muscle mass, there is a mathematical imprecision. The MSD Manual quantifies it clearly: Creatinine clearance exceeds the actual filtration capacity by about 10 to 20 percent in normal kidney function and by up to 50 percent in advanced kidney failure (as of 2025).

The reason is a secondary pathway. In addition to filtration, some creatinine is actively released into the urine. The poorer the kidneys' filtration, the more this secondary pathway matters—and the more flattering the calculation looks.

Creatinine, cystatin C, and the combination compared

There are three ways to estimate the kidneys' filtration capacity from the blood. The table compares them according to the same four criteria.

The last line is the actual finding of this chapter. Cystatin C does not replace creatinine; it confirms or refutes it. Galle describes cystatin C as an early-rising marker of impaired kidney function and classifies the combined formula as an improvement recommended for confirmatory testing (2016).

The third line in the cystatin C column needs an addition. The MSD Manual notes that tests are not always available and that laboratories do not have standardized values (as of 2025). Two results from two laboratories are therefore less readily comparable for cystatin C than for creatinine.

How a laboratory value becomes an estimated filtration capacity

The report rarely lists only the concentration. Usually, a second figure appears next to it, with the unit milliliters per minute. This is the estimated glomerular filtration rate, or eGFR for short—the amount of blood the kidneys clean per minute.

The Institute for Quality and Efficiency in Health Care explains the detour matter-of-factly: Measuring the filtration rate directly would be very labor-intensive, so it is usually estimated, for example using the creatinine level in the blood (as of August 14, 2024).

The rest is a formula. It takes the laboratory value and factors in age and sex, because both influence the expected muscle mass. A calculation using cystatin C does not need these correction factors in the same way.

What the unit ml/min/1.73 m² means

The unit looks unwieldy and can be explained in one sentence. Milliliters per minute is the amount of blood cleaned. The addition of 1.73 m² is a calculated standard body surface area to which the value is converted.

Without this conversion, test results would not be comparable. A large, heavy person has larger kidneys and filters more blood in absolute terms than a small, light person. Only standardization makes two very different bodies into two comparable figures.

For you as the reader, this mainly means one thing. The value on your test result has already been converted, so you do not need to do anything with it, and you can compare it directly with the figures in the next chapter.

Why test results often show two eGFR values

When both are measured, the laboratory often provides three figures: an eGFR based on creatinine, one based on cystatin C, and one based on the combination of the two. If the first two differ widely, that is not a contradiction but information.

A significantly lower cystatin C estimate with a good creatinine value fits the situation Galle describes: low muscle mass, deceptively low creatinine, and filtration capacity that is actually lower than the first value suggests.

Why the middle number is the most important

The difference between 130 and 75 represents roughly 45 percent of filtration capacity, and this decline is part of normal aging. A value that would be cause for concern in a seventy-year-old if measured against the standard for a thirty-year-old may be unremarkable for their age.

That is why the threshold of 60 is linked to a time period. The MSD Manual considers it chronic kidney disease only when this value remains present for more than three months (as of 2025). A single measurement on a single day never meets this condition.

What the comparison with creatinine clearance shows

The Institute for Quality and Efficiency in Health Care gives a creatinine clearance range of 95 to 160 ml/min/1.73 m² for women and 98 to 156 for men aged over 18 (as of March 20, 2025). These figures are visibly higher than the estimated filtration rate values.

This is not a contradiction, but precisely the 10 to 20 percent excess described in the MSD Manual. Two numbers on the same report can refer to the same kidney function and still differ in magnitude because they are obtained in different ways.

Chapter at a glance

Kidney filtration capacity is 120 to 130 ml/min/1.73 m² in young, healthy adults and about 75 at age 70, even in the absence of disease. Chronic kidney disease is defined as a value below 60 for more than three months. A single measurement cannot meet this condition because it lacks a time component. Reference ranges for creatinine clearance are higher than the estimated glomerular filtration rate because clearance exceeds actual filtration capacity by 10 to 20 percent.

When cystatin C makes a difference

In a muscular thirty-year-old man with unremarkable findings, both values will generally convey the same message. The second value is worthwhile where the first can systematically be misleading.

Low muscle mass with normal creatinine

This is the case described in the quotation above. A very slender person with little muscle mass may have creatinine in the middle of the reference range and still have a filtration rate well below what that number suggests.

A normal creatinine value is not proof of healthy kidneys. It is an indication whose significance rises and falls with muscle mass.

Older age

Muscle mass decreases with age, while filtration capacity also declines as a matter of course. Both changes move in the same direction and partially offset each other in the creatinine value. The value remains stable while two things are changing simultaneously.

After prolonged bed rest or illness

Galle explicitly mentions bedridden people. Anyone confined to bed for weeks loses muscle mass, and the creatinine value follows this change rather than kidney function.

Very high muscle mass

The situation can also work the other way around. Someone with substantially more muscle mass than average produces more creatinine, making the estimated filtration rate appear too low. The finding looks worse than the kidneys are functioning.

This applies to strength athletes and anyone who performs heavy physical work. A value that prompts further investigation often has a harmless explanation in this context, and cystatin C can support or rule out that explanation.

When two findings do not match

An abnormal urinalysis despite normal creatinine is a reason to look more closely. The same applies to a creatinine value that fluctuates markedly from one measurement to the next without any change in lifestyle.

In both cases, a second value obtained in a different way provides the decisive information. If creatinine and cystatin C agree, the first value was reliable. If they differ, the discrepancy itself is the finding.

What else shifts the value

No kidney value is free from confounding factors. For creatinine, the best-known one is diet. Meat contains creatine, which is converted into creatinine in the body, and a large meat meal before a blood draw can temporarily raise the level.

With cystatin C, the confounding factors are different. Although its concentration is independent of sex, age, and body weight, it is not independent of everything. Thyroid function, inflammatory processes, and certain medications are described in the scientific literature as influencing factors.

The wording here is deliberately cautious. These influences are described and discussed in the scientific literature; we did not find a reliable figure in the sources reviewed indicating by what percentage they shift a cystatin C value. That is why there is no figure here.

Another confounding factor affects both values equally: the amount of fluid in the body. Anyone who goes for a blood draw severely dehydrated has more concentrated blood, and both kidney values are higher, even though nothing has changed in the kidney.

This leads to a simple rule for both values. A single measurement describes a condition on the day the blood sample is taken. Only a repeat measurement after a few weeks shows whether that condition persists.

Three statements about kidney values, fact-checked

The following three statements come up regularly when it comes to kidney values. All three are so widespread that they are rarely questioned.

The second point needs a qualification so that the warning does not cause panic. Being symptom-free does not mean that something has been overlooked. It means that early stages are detected through values rather than symptoms—and that is precisely what these values are for.

A measurement becomes meaningful when there is a reason for it. The following four points describe what needs to be clarified beforehand.

Step 1

Look up your previous results

A kidney value without any medical history says little. Two values measured months apart say considerably more.

Step 2

Choose the timing deliberately

Not directly after a large meat-heavy meal and not the day after a very strenuous workout.

Step 3

Make a note of your medications

Long-term medication belongs on the same sheet as the result. Without it, half the basis for interpretation is missing.

Step 4

Have abnormal results assessed by a doctor

Foamy urine, blood in the urine, swollen legs, or a decrease in urine output belong in a doctor’s office, not in a self-test.

Limits: what a single value does not answer

A cystatin C value does not explain why the filtration rate is what it is. It measures an outcome, not a cause. No single lab value can distinguish between high blood pressure, diabetes, a medication effect, and kidney disease in the narrower sense.

The second limitation is the time frame. The threshold of 60 mL/min/1.73 m² in the MSD Manual is tied to a duration of more than three months (as of 2025). A single measurement cannot meet this requirement, and an abnormal isolated value is therefore a reason to repeat the test, not to draw conclusions.

The third limitation concerns comparability. Because, according to the MSD Manual, there are no standardized cystatin C values across laboratories, two reports from two different facilities cannot readily be compared side by side (as of 2025). The relevant reference range remains the one shown on your own report.

And the fourth limitation concerns what is not found in the blood at all. Protein in the urine is an independent sign of kidney damage and is detected through a urine sample, not a blood sample. A good blood value does not replace this test.

The objection that listing limitations downplays the test’s usefulness is justified. The answer lies in the evidence itself. A value that detects reduced filtration early is valuable precisely when it is treated as a starting point, not a verdict.

This list reads like an argument against testing. It is not. It is an argument against expecting one number to answer a question that requires two tests and a time interval.

What matters about cystatin C

If you take away one action from this article, let it be this: Check your latest lab report to see whether an estimated filtration rate is listed alongside the creatinine, and write down both numbers and the date on a sheet of paper. The next report adds another line.

The reason is unremarkable. Kidney function is not assessed by a single value, but by how it changes. Two lines on a sheet of paper do more for this than any individual measurement, and they cost nothing.

The starting question was what cystatin C shows. The most honest answer is: the same thing as creatinine, just without its blind spot. That is why it is used alongside it rather than in its place.
