Strictly speaking, vitamin D is a hormone that the body produces itself in the skin under the influence of UV-B radiation. The storage form, 25-OH vitamin D3, is measured because it reliably reflects vitamin D status over several weeks. Vitamin D regulates calcium absorption from the intestines and also affects the muscles and immune system.

Why is this value measured?

In Germany, sunlight from October to March is insufficient to produce enough vitamin D. Deficiencies are therefore widespread and usually go unnoticed because the symptoms – fatigue, muscle weakness, and susceptibility to infections – are nonspecific.

Too high

Very high levels practically only result from overdosed supplements and can cause harm through elevated calcium levels. An overdose from sunlight alone is not possible.

Too low

Low levels promote bone loss, muscle weakness, and susceptibility to infections. At-risk groups include older people, people with dark skin, shift workers, and anyone who spends little time outdoors.

When should vitamin D levels be measured?
The most informative time to measure it is at the end of winter, in February or March, when levels are at their lowest.

How long does it take for the level to improve?
With regular supplementation, levels rise over approximately eight to twelve weeks. A follow-up measurement is advisable no earlier than three months later.

Is summer sunlight enough?
For people with fair skin, 10 to 20 minutes of midday sun on the face and forearms is often enough in summer. In winter, the sun is too low in Germany for significant vitamin D production.

Alanine links muscle and liver metabolism. During physical exertion or fasting, the muscle releases nitrogen in the form of alanine. The liver uses it to produce new glucose – the so-called glucose-alanine cycle, which keeps blood sugar stable even during fasting.

Why is this value measured?

Albumin reflects liver function, nutritional status, and kidney function in a single value. In aging research, well-preserved albumin is considered one of the most stable markers of physical resilience in older age. It is also needed to calculate free testosterone and corrected calcium.

Too high

Elevated levels are almost always caused by dehydration and do not constitute an independent disease.

Too low

Low levels occur with liver disease, protein loss through the kidneys or intestines, malnutrition, and chronic inflammation.

Why is albumin needed to calculate testosterone?
Some testosterone is loosely bound to albumin and remains biologically available. Albumin and SHBG are therefore needed to calculate free and bioavailable testosterone.

Arginine is the precursor to nitric oxide, a messenger that relaxes the smooth muscle in blood vessel walls, thereby dilating the vessels. This lowers blood pressure and improves blood flow to muscles and organs. Arginine is also part of the urea cycle, through which the body detoxifies ammonia.

No. The medical term describes a measurable shift in blood pH and refers to a clearly defined condition. It is not synonymous with the concept of generalized tissue acidification.

Why is this value measured?

Because the body keeps the blood level constant at all costs, an abnormal calcium level usually indicates a disorder of the regulatory systems themselves—for example, the parathyroid glands, kidneys, or vitamin D metabolism.

Too high

Elevated levels may indicate overactivity of the parathyroid glands, high-dose vitamin D supplements, or tumors and should always be investigated.

Too low

Low levels are caused by vitamin D deficiency, kidney disease, magnesium deficiency, or underactive parathyroid glands. Typical symptoms include tingling in the fingers and around the mouth, as well as muscle cramps.

Does the calcium level indicate my bone density?
No. The blood level remains normal for a long time even as bone loss progresses because the body supplies calcium from the bones. Bone density requires measurement by DXA.

Why is calcium measured together with albumin?
Some calcium is bound to albumin. When albumin is low, total calcium is lower without there being a true deficiency; the value is then mathematically corrected.

Chloride is the most common negatively charged particle in the blood and accompanies sodium in almost all regulatory processes. It maintains the electrical neutrality of the blood, is involved in fluid balance, and provides the basic substance for stomach acid.

Why is this value measured?

Is chloride assessed on its own?
No. Chloride must always be considered in conjunction with sodium, potassium, and bicarbonate. Only this combination allows conclusions to be drawn about acid–base balance.

Citrulline is part of the urea cycle and is converted into arginine in the kidneys. Unlike arginine, it is not already broken down in the intestines and liver, so ingested citrulline raises blood arginine levels more reliably—and therefore also increases nitric oxide production. Why does citrulline work better than arginine?
Arginine is largely broken down in the intestines and liver. Citrulline passes through both organs and is only converted into arginine in the kidneys, causing arginine levels to rise more reliably.

What does citrulline indicate about the intestines?
Citrulline is produced in the cells of the small intestinal lining. A low level is considered an indication of a reduced mass of functioning intestinal cells.

Cortisol is produced in the adrenal cortex and follows a pronounced daily rhythm: It is highest shortly after waking and lowest around midnight. It mobilizes energy, maintains blood pressure, and dampens inflammation. Without cortisol, the body cannot cope with physical stress.

Why is this value measured?

Cortisol helps interpret symptoms such as persistent fatigue, sleep disturbances, muscle loss, and increased abdominal fat. Because of its daily rhythm, the time of blood sampling is crucial to the meaningfulness of the result.

Elevated levels occur with acute stress, corticosteroid medication, pregnancy, use of the contraceptive pill, and Cushing's syndrome.

Why does cortisol have to be measured in the morning?
The level fluctuates several-fold over the course of the day. Only a sample taken early in the morning allows meaningful interpretation based on the reference ranges.

Does a normal cortisol level show that I am not stressed?
No. A single measurement captures a snapshot. To assess chronic stress, profiles over the course of the day are more informative.

C-reactive protein is a protein produced in the liver when inflammation occurs in the body. The level rises measurably within 6 to 12 hours after the onset and falls just as quickly once the inflammation subsides. This makes it suitable for detecting inflammatory processes and monitoring the course of treatment.

Why is this value measured?

CRP shows whether inflammation is active in the body—even when no symptoms are noticeable yet. In its high-sensitivity form, the level is also used to assess long-term cardiovascular risk.

Too high

Significantly elevated levels suggest an acute bacterial infection, an injury, or an inflammatory disease. Slightly elevated levels can occur with viral infections, overweight, smoking, or strenuous physical activity.

Too low

Low levels are normal and considered unremarkable. At the very beginning of an infection, however, a low CRP level does not rule out inflammation.

What does an elevated CRP level mean?
It indicates that inflammation is occurring in the body. In bacterial infections, the increase is usually much greater than in viral infections.

What is the difference between CRP and hs-CRP?
The same substance is measured, but with different measurement sensitivities. The highly sensitive hs-CRP also detects very low concentrations and is used to assess the risk of cardiovascular disease.

Cystatin C is a small protein produced uniformly by all nucleated cells. It is filtered exclusively through the kidneys and completely broken down there. Unlike creatinine, its level does not depend on muscle mass and therefore provides more reliable results in very muscular or very lean people.

Why is this value measured?

Which is better: creatinine or cystatin C?
Cystatin C is independent of muscle mass and is therefore more reliable in strength athletes, very lean people, and older adults. The most accurate estimate of the filtration rate is calculated by combining both values.

Does strength training affect the level?
No. That is precisely the advantage over creatinine, which can be elevated in people with a lot of muscle mass even when the kidneys are not impaired.

Why is this value measured?

Why must DHEA-S be assessed in relation to age?
The level declines continuously from around the age of 25. A value can therefore only be meaningfully interpreted by comparison with the appropriate age group.

Serum iron measures the small amount of iron currently circulating in the blood, bound to the transport protein transferrin. The level fluctuates considerably throughout the day and responds immediately to the last meal or tablet taken. It is therefore of limited significance on its own and is always assessed together with ferritin and transferrin.

Why does iron decrease when you have a cold?
The body deliberately deprives pathogens of iron by moving it from the blood into storage. A low level during an infection therefore does not automatically indicate a deficiency.

Why is this value measured?

In women, estradiol helps assess menstrual irregularities, infertility, and menopausal symptoms. In men, an elevated level explains symptoms such as breast enlargement, fluid retention, and loss of libido—often associated with overweight.

Too high

Physiologically normal in women around the middle of the menstrual cycle. Persistently elevated levels occur with overweight, liver disease, and certain cysts. In men, overweight is the most common cause because adipose tissue converts testosterone into estradiol.

Too low

In women, low levels indicate menopause, being severely underweight, competitive sports, or a disorder of the pituitary gland. In men, levels that are too low impair bone density and libido.

For basic testing on the third to fifth day of the menstrual cycle, together with FSH and LH. Other testing times may be appropriate for other questions.

Why is estradiol also important for men?
Men need estradiol for bone density, libido, and normal fat metabolism. Both excessively high and low levels cause symptoms.

Ferritin is the storage protein for iron. It binds iron in the liver, spleen, and bone marrow and releases it when needed. The ferritin level in the blood approximately reflects how well the iron stores are filled—and falls long before the hemoglobin level drops.

Ferritin detects iron deficiency at an early stage, before anemia develops. It can often explain typical symptoms such as fatigue, hair loss, concentration problems, or shortness of breath.

Too high

Elevated levels result from inflammation, liver disease, high alcohol consumption, or iron overload. Because ferritin is also an acute-phase protein, it should always be assessed together with CRP.

Too low

Low levels indicate iron deficiency. Common causes include heavy menstrual bleeding, an iron-poor diet, endurance sports, pregnancy, or blood loss in the gastrointestinal tract.

At what ferritin level is iron deficiency diagnosed?
Levels below 15 µg/l are considered definitive iron deficiency. Many professional societies already consider levels below 30 µg/l to require treatment when typical symptoms are present.

Why is my ferritin normal even though I have symptoms?
With simultaneous inflammation, ferritin rises regardless of iron status and can mask a deficiency. Therefore, CRP is also measured.

How long does it take to replenish iron stores?
Oral iron therapy generally takes three to six months, even though symptoms often improve after just a few weeks.

Folate is the natural form of vitamin B9 and is needed for every cell division. The need is particularly high where tissue renews rapidly: in the bone marrow during blood formation and in early embryonic development. Together with vitamin B12, folate also breaks down homocysteine.

Why is this value measured?

Folate deficiency causes anemia with enlarged red blood cells, which can easily be mistaken for vitamin B12 deficiency in a blood count. When trying to conceive, good nutritional status is crucial even before conception.

Too high

High levels result from supplements and are generally harmless. However, they can mask a simultaneous vitamin B12 deficiency in the blood count.

Too low

Causes include a diet low in vegetables, alcohol consumption, chronic intestinal diseases, pregnancy, and certain medications.

When should folic acid supplementation be started when trying to conceive?
It is recommended to start at least four weeks before conception, as the neural tube closes during the first weeks of pregnancy—often before the pregnancy is known.

What is the difference between folate and folic acid?
Folate is the natural form found in food, while folic acid is the synthetic form in supplements. Folic acid is absorbed better, but the body must first activate it.

Only about one to two percent of testosterone circulates unbound in the blood and can attach directly to cells. This free fraction is not measured directly but calculated from total testosterone, SHBG, and albumin. The result reflects the actual hormonal effect much better than the total value alone.

Why is this value measured?

When SHBG is elevated—for example in older age, with hyperthyroidism, or while taking estrogen—total testosterone may appear normal, while free testosterone is already significantly too low. The calculated value explains precisely these cases.

Elevated levels occur when SHBG is low, for example with overweight, insulin resistance, or polycystic ovary syndrome.

Too low

Low levels despite normal total testosterone indicate elevated SHBG and often explain symptoms that could not be inferred from the total value.

Why is free testosterone calculated instead of measured?
Direct measurement is complex and prone to interference. Calculation from total testosterone, SHBG, and albumin is considered more reliable and is the international standard.

FSH is produced in the pituitary gland and acts directly on the gonads. In women, it stimulates the follicles to grow; in men, it controls sperm production in the testes. When gonadal function declines, FSH levels rise—the body then tries to compensate more strongly.

Why is this value measured?

Too low
