What albumin actually does in the blood

Albumin is a protein that the liver produces and releases into the blood. By quantity, it is the largest single component among blood proteins, which is exactly why it appears on many laboratory reports.

When someone reads the term on a test report for the first time, they usually look for a simple classification: high is good, low is bad. That is not how this value works. It is a measure that depends on several parts of the body at the same time.

How the value is measured

Albumin in the blood is measured in the laboratory from a blood sample and reported in grams per liter. It is therefore one of the values that requires no special preparation and is routinely included in many standard test orders.

The measurement can be taken from either venous blood or capillary blood from a finger prick. When comparing two measurements, the same rule applies as for other laboratory values: anyone who wants to see a change should stick to one method and one laboratory.

One point deserves attention because it influences the value more than many other factors: body position during blood collection. Anyone who has been lying down for an extended period has more fluid in the blood vessels, so the measured value is slightly lower than when standing. This alone can create a difference between a measurement in the hospital and one at home.

In practice, this is nothing dramatic. It simply means that a deviation of a few percent between two results is not an event, but the normal variation of a value linked to fluid balance.

Transport and fluid balance: the two functions

The second function is less well known and more important for understanding a low level. Albumin keeps water in the vascular system because dissolved proteins bind fluid.

If the amount of albumin in the blood drops significantly, fluid shifts into the surrounding tissue. This becomes visible as fluid retention, typically first in the lower legs. This is not proof of a low albumin level, but it is a sign that may point in the same direction.

The sentence describes the normal situation: Albumin remains in the blood because the kidneys retain it. This also leads to the first possible explanation for a low level, and it appears in Chapter 6.

What a low albumin level can mean

Because the value depends on several factors, there is no single explanation. There are four directions it can point in, and they are not mutually exclusive.

Direction one: Less is produced

Albumin is produced in the liver. If liver function is impaired, production may decrease. A low albumin level is therefore assessed in laboratory diagnostics together with liver values and never on its own.

Direction two: Something is being lost

The second pathway involves the kidneys. If they are damaged, according to the IQWiG, more blood proteins are excreted, and the level of protein in the urine can be used to estimate how severe the kidney damage is (2024).

Losses are also possible through the intestines and through extensive skin damage. In all these cases, production is functioning properly; the protein simply does not remain in the blood.

Direction three: The building material is missing

A markedly inadequate protein intake over a prolonged period can also affect the value. This concerns less the average diet in Germany than situations involving severely restricted food intake.

Direction four: It is diluted or redistributed

The fourth pathway is the least concerning and is overlooked most often. During acute inflammation, the body temporarily shifts its protein production, and fluid balance also affects the measured value.

That is why the same applies to albumin levels as to laboratory values in general: Many factors can influence them, and there are often harmless reasons for this (IQWiG, 2025). A single measurement during an infection describes the infection phase.

And what if the value is elevated?

An elevated albumin level occurs much less often than a low one, and the simplest explanation is the most common: too little fluid in the blood vessel. When the blood becomes more concentrated due to dehydration, all the substances dissolved in it increase proportionally.

This is the same mechanism as in direction four, only in the opposite direction. Anyone who drank little, sweated heavily, or had diarrhea on the day of the blood draw is measuring an effect of their fluid status, not of protein production.

This leads to an unspectacular recommendation: Drink as you would on a normal day, neither particularly much nor particularly little. Anyone who changes their fluid intake shortly before the sample changes precisely the factor on which this value depends.

A persistently elevated value without such an explanation is assessed in a medical practice, just like a low one. The same applies here: The number alone does not identify a cause.

Why the trend says more than a single value

Albumin remains in the blood for a comparatively long time. This makes the value sluggish: it does not respond to yesterday's lunch or to one bad night's sleep.

This sluggishness is an advantage in everyday life. It means that a change over several weeks describes something that persists longer, rather than a single day's condition. That is precisely why the second measurement is more informative for this value than it is for some others.

But it also has a downside. Because the value changes slowly, an early change does not show up in the number until later. A normal albumin value therefore does not rule out that something is changing in one of the four areas.

For interpreting a test result, this means that the albumin value is not an early warning value but a measure of the amount present. It describes a situation that has developed over several weeks.

These three statements appear in many forums and advice articles. All three sound plausible, and all three stand up to scrutiny only partially.

This list does not replace an examination, nor is it a diagnostic grid. It marks the point at which a lab value, without context, is no longer helpful.

A single point from this list means little on its own. Swelling around the ankles after a long flight has a harmless explanation. The picture becomes noteworthy when two points apply at the same time and persist for weeks.

Albumin in the blood and albumin in the urine are two different questions

This is where the most common confusion about this topic arises. Both tests contain the same word in their names but answer different questions.

The blood value indicates how much albumin is present in the bloodstream. The urine value indicates how much of it is being lost. A low blood value combined with an abnormal urine value presents a different picture from a low blood value with unremarkable urine.

What the urine value additionally shows

According to IQWiG, the level of protein in the urine can be used to estimate how severe kidney damage is (2024). This gives the value a degree of severity that the blood value alone does not provide.

Kidney function provides additional information. The glomerular filtration rate indicates how much blood is filtered per minute; in healthy people, according to the same source, this is between 85 and 135 milliliters per minute (2024).

A home urine test does not replace this assessment. An elevated protein level in the urine, known as proteinuria, should be medically evaluated because elevated protein levels can indicate, among other things, inflammation of the kidneys (IQWiG, 2023).

Why a test strip does not answer the question

Urine test strips from pharmacies test for protein, among other things, and the normal result for this field is negative (IQWiG, 2023). Such a strip is a rough screening test, not a measurement.

Two limitations follow from this. First, a strip test does not reliably detect small amounts of protein, and early losses are particularly small. Second, the result depends on the concentration of the urine: anyone who has drunk a lot dilutes their urine, and therefore also what the strip could detect.

That is why a medical practice does not simply test for protein; it measures the amount and relates it to a second value. Only this ratio makes two urine samples comparable, regardless of how much someone drank beforehand.

For you, this means that a negative strip test is not an all-clear, and a positive one is not a diagnosis. Both are reasons to take the question to the place where it can be answered.

Why albumin is needed to determine free testosterone

This relationship is why albumin appears on hormone profiles even though it is not itself a hormone.

Most testosterone in the blood is bound to two transport proteins: SHBG and albumin. Only a small proportion is unbound, and this proportion is called free testosterone.

Because the free fraction cannot readily be determined directly, it is calculated in many profiles. Three values go into this calculation: total testosterone, SHBG, and albumin. If one is missing, there is no result.

This leads to a practical insight when reading a hormone report: The albumin value is not presented there as an independent question but as a calculation variable. The differences between the two transport proteins are explained in SHBG: what the value says about available hormones.

Why SHBG fluctuates more than albumin

Of the two transport proteins, albumin is the steadier one. It binds testosterone more loosely, but is present in large amounts and changes little in everyday life.

SHBG behaves differently. It binds more tightly, occurs in a considerably smaller amount, and responds to more influences. Therefore, a change in free testosterone is more often explained by SHBG than by albumin in the calculation.

For interpreting a hormone report, this means: The albumin value is usually the stable part of the equation. If it nevertheless deviates significantly, it is worth asking whether it fits with the other values or points to something else.

Chapter at a glance

On a hormone profile, albumin plays a different role than on a liver or kidney report. There, it is not an independent question but one of three variables used to calculate free testosterone: total testosterone, SHBG, and albumin. The reference range of your own laboratory matters, not a number from the internet.

Step 2

Note the circumstances

Infection, fever, surgery, or a strict diet in the weeks before the sample was taken.

Step 3

Bring along the neighboring values

Liver values, kidney values, and the inflammation marker are usually on the same sheet.

Step 4

Ask a specific question

Instead of “Is it serious?” ask: Does this value fit with my other values?

The third step is the one that helps most. An albumin value without the values alongside it requires a new blood draw; a complete report often does not.

Questions that help move the conversation forward

Anyone who goes to a medical practice with a test result gets better answers when they ask better questions. Three of them are almost always useful.

The first: Does this value fit with my liver and kidney values on the same sheet? This shifts the conversation from an isolated number to the context in which it appears.

The second: Should the protein in the urine be measured? This question closes the gap between stores and loss discussed in Chapter 6, and it can be answered with a single urine sample.

The third: When should it be checked? A single value outside the reference range gains its significance from a second measurement, and a set interval is better than a vague Later.

These three questions do not replace an examination. They ensure that the twelve minutes an appointment often takes are used for what matters.

Which values besides albumin appear on the test report

Three protein values are regularly confused because they have similar names but describe different things. The table compares them according to the same four criteria.

Once these three have been distinguished, a finding is read differently. The question is no longer whether a number is too low, but which of the three questions it actually answers.

What can be inferred from the combination

The benefit of this distinction becomes apparent only when two of the three values are available. A low albumin value with normal urine protein largely rules out the route through the kidneys and directs attention to production, absorption, or dilution.

Conversely, a low albumin value together with an abnormal urine value directly indicates a loss. Neither is a diagnosis; they are directions that help shorten the investigation.

Total protein provides a third perspective. If it decreases in parallel with albumin, the change affects overall protein status; if it remains normal while albumin decreases, the distribution between the protein groups has shifted.

These distinctions are made in a medical practice, not in an article. But understanding them explains why a test request rarely lists only one value.

Limitations: what the value does not answer

The first limitation is fundamental: An albumin level does not identify a cause. It shows that the amount in the blood differs from what is expected, while leaving open which of the four points in Chapter 3 is responsible.

The second limitation concerns the numbers. IQWiG provides specific figures for kidney function and protein excretion. There is no comparable figure from a German institution for how often a low albumin level has a particular cause. That is why this article does not include one.

The third limitation is stated in the product cards: These tests measure albumin in the blood. Urine albumin and kidney function are separate tests and are not included there.

And a fourth factor that matters particularly for this value: Reference ranges depend on the laboratory, method, and age. Two reports from two laboratories therefore cannot be compared number for number, only against each laboratory's own range.
