Understanding liver values: which ones count and what they show

The essentials at a glance

The commonly used liver values measure two different things. ALT and AST show whether liver cells are being damaged: they leak out of damaged liver cells and, according to the MSD Manual Professional Edition, are sensitive indicators of such damage (as of November 2025). Gamma-GT and alkaline phosphatase, on the other hand, indicate whether bile flow is impaired.

None of these values answers a question on its own. Only the pattern formed by several values helps narrow down whether the issue concerns the cells, bile flow, or neither. And the reference ranges themselves are less clear-cut than their presentation on the lab report suggests.

Which values count as liver values

The term liver values is a collective term. It refers to several measurements from the blood that are related to the liver and bile ducts in different ways.

The first group consists of enzymes from liver cells. The MSD Manual Professional Edition states that alanine aminotransferase and aspartate aminotransferase leak out of damaged liver cells and are therefore sensitive indicators of cell injury (as of November 2025).

The second group indicates bile flow. It includes alkaline phosphatase and gamma-glutamyl transferase (GGT). If both rise, this suggests a buildup of bile rather than primarily dying liver cells.

What else appears on the test report

In addition to the four enzymes, two other measures may appear that answer a different question. Bilirubin is a breakdown product that the liver processes and excretes in bile. Albumin and blood clotting describe what the liver produces.

The difference is fundamental. Enzymes in the blood indicate that something is being damaged or that a blockage is occurring. Low albumin or prolonged clotting time indicates that the liver’s production function is declining.

The MSD Manual illustrates how rarely this occurs in metabolic fatty liver disease: hyperbilirubinemia, prolonged clotting time, and low albumin are rare in such cases (as of August 2025). These values therefore become abnormal only at a late stage, which is precisely why the enzymes provide an earlier indication.

The German and international names

Two naming systems appear side by side on test reports, which regularly causes confusion. GPT is the same measurement as ALT or ALAT, namely alanine aminotransferase. GOT is the same measurement as AST or ASAT, namely aspartate aminotransferase.

Gamma-GT also has several other names, including GGT and gamma-glutamyl transferase. Anyone comparing two test reports from different laboratories should therefore first match the names and then compare the numbers.

The four most important values compared

Four values appear on almost every test report concerning the liver. The table compares them according to the same four criteria.

The second column explains a common misconception. Many people regard GOT as a liver value, and the MSD Manual states that the enzyme is found in the heart, skeletal muscle, kidneys, red blood cells, and pancreas (as of November 2025). An elevated GOT therefore does not necessarily come from the liver.

Why no single value proves anything

The table leads to a rule that underpins every evaluation. Each of the four values has at least one other possible source, and therefore the first question is never how high a value is, but which values stand out together.

The MSD Manual makes this particularly clear for alkaline phosphatase. A hepatic origin of an elevated level must be confirmed by a simultaneous elevation of 5′-nucleotidase or gamma-glutamyltransferase (as of August 2025).

A value is therefore confirmed by a second one. This is exactly how a liver panel is evaluated, and precisely what a single number on a printout cannot show.

Two patterns that can be distinguished

In practice, the two groups produce two typical patterns. In the first, GPT and GOT are clearly above the threshold, while Gamma-GT and alkaline phosphatase are barely noticeable. This points to a process within the liver cells themselves.

In the second pattern, the opposite is true. Gamma-GT and alkaline phosphatase rise together, while the aminotransferases remain close to the threshold. This points to impaired bile flow.

There are also mixed patterns, and they are not unusual. They are why a medical practice asks further questions after seeing the values rather than immediately assigning a diagnosis.

What a normal value actually is

The reference range on your report describes what is typical in a healthy comparison group. It does not describe the value at which something is no longer right, and these two statements do not always coincide.

The MSD Manual, Professional Edition, summarizes in a sentence rarely heard how far apart they can be for alanine aminotransferase.

The Institute for Quality and Efficiency in Health Care cites reference ranges for the same measurement of below 35 U/L for women aged 18 and over and below 50 U/L for men aged 18 and over (as of March 20, 2025). Both figures are shown side by side here because they differ.

We do not resolve this difference. It is real, documented in both sources, and the best reason not to consider a value in the upper range settled simply because there is no arrow next to it.

How high a value is elevated also matters

On the report, every exceedance looks the same. In the assessment, it is not, and the MSD Manual draws the line at a clear point.

For mild, isolated elevations of ALT or AST, meaning less than twice the normal value, the test should initially be repeated. In about one third of cases, the value normalizes (as of August 2025).

One third is a lot. This figure alone justifies repeating the test as a first step and explains why a medical practice often does not immediately order further tests when a value is just slightly elevated.

Why the multiple matters, not the difference

The medical literature calculates liver values as multiples of the upper limit of normal, not as absolute differences. A value at one and a half times the upper limit is treated differently from one at ten times the upper limit, regardless of how many units lie between them.

For alkaline phosphatase, the MSD Manual cites two such benchmarks. Increases up to three times the normal value occur in many liver diseases, while the level rises to four times or more one to two days after the onset of bile duct obstruction (as of November 2025).

The same source also explains why an elevated value remains elevated for a while after the underlying cause: The half-life of alkaline phosphatase is approximately seven days. A value may therefore still be elevated even when the problem has already been resolved.

There is a reason for the fasting rule

The same source specifies a practical condition for testing. Laboratory tests should be performed while fasting because oral intake leads to slight increases (as of August 2025).

So anyone who measures after breakfast has caused part of the increase themselves. This is not a major effect, but when a value is just above the limit, it determines the arrow on the printout.

The upper limits shown

A comparison for men shows how far the figures actually differ. The first two bars come from the IQWiG reference ranges, and the third from the MSD Manual.

The gap between the second and third bars is 17 units. In this range, one source considers the result within the normal range, while the other considers it above normal.

In practical terms, this does not mean that you should apply the lower standard. It means that a value at the upper end of the reference range is worth monitoring and does not provide reassurance.

What Temporarily Shifts Liver Values

Three things can shift liver values without anything changing in the liver. The first is the meal before the test, the second is a medication, and the third is physical exertion.

There is documented information on the third point. The Institute for Quality and Efficiency in Health Care states that strenuous exercise can temporarily increase alanine aminotransferase levels (as of March 20, 2025).

The Ratio Between the Values

Two values can reveal more than either one alone. The MSD Manual gives an example that relates to the everyday question of alcohol.

In most liver diseases, the ratio of aspartate aminotransferase to alanine aminotransferase is below 1. In alcohol-related liver disease, it is typically above 2 (as of November 2025).

For metabolic fatty liver disease, the same source gives the ratio in the other direction: in the inflammatory form, the ratio of alanine aminotransferase to aspartate aminotransferase is generally below 1 (as of August 2025).

Chapter at a Glance

The ratio of two liver values provides more information than the level of either one alone. According to the MSD Manual, the ratio of aspartate aminotransferase to alanine aminotransferase is below 1 in most liver diseases and typically above 2 in alcohol-related liver disease. In the inflammatory form of metabolic fatty liver disease, the reverse ratio is generally below 1. A medical practice assesses such ratios alongside the complete test results, not a calculator on the internet.

What Bilirubin Adds

Bilirubin is not an enzyme but a breakdown product of the red blood pigment. It appears on the test report alongside the enzymes because the liver takes it up, processes it, and excretes it in bile.

The MSD Manual states that total bilirubin is normally present largely in unconjugated form, with values below 1.2 mg/dl (as of November 2025). The distinction between the conjugated and unconjugated fractions is the key information.

The same source sets the dividing lines at an indirect bilirubin proportion above 85 percent for the unconjugated form and a direct proportion above 50 percent for the conjugated form (as of November 2025). Only this breakdown indicates whether the cause is before the liver, in the liver, or beyond it.

Why a high bilirubin level can also originate in the blood

Bilirubin is produced when red blood cells break down. If more red blood cells are broken down than usual, more bilirubin is produced and the level rises without the liver being involved.

For this reason, the MSD Manual recommends checking hemoglobin when investigating an isolated increase in bilirubin, in order to rule out increased breakdown of red blood cells (as of August 2025).

For congenital disorders of bilirubin metabolism, the same source gives an approximate range: The values are typically below five times the normal value, that is, below 6 mg/dl (as of November 2025). This is another example of how a value only becomes meaningful when considered in terms of its magnitude.

Three statements about liver values in a fact check

The following three statements come up regularly when someone reads their test results. All three can be checked against the evidence.

The third point warrants some context so that it does not cause confusion. Both figures are supported by evidence, and neither is a diagnostic threshold. What matters for your result is the range reported by the laboratory evaluating it.

What a capillary blood test can show here

A capillary blood self-test provides individual values, not a complete liver profile. It shows where these values stand on a given day but does not interpret them.

Four points determine how much a measured liver value ultimately tells you.

The number alone says nothing. It can only be interpreted together with the reference range used by the evaluating laboratory.

Yellowing of the skin or eyes, pale stools, dark urine, or upper abdominal pain require immediate medical attention.

Limitations: what liver values leave unanswered

The first limitation is scope. For persistent or marked elevations, the MSD Manual lists a whole series of investigations, including hepatitis B and C, iron-storage disease from age 40, Wilson disease under age 30, autoimmune diseases, alpha-1 antitrypsin deficiency, celiac disease, and drug-induced liver injury (as of August 2025).

The second limitation is the view of the organ itself. The same source lists an abdominal ultrasound as part of the diagnostic process and mentions a liver biopsy as an option to consider if the entire series of examinations remains inconclusive.

The third limitation is the time dimension. A single measurement cannot distinguish between a temporary and a persistent elevation, and in around one-third of cases, a mildly elevated value returns to normal when repeated.

The fourth limitation is the reference range itself. As long as two reliable sources give different upper limits for the same measurement, the boundary between normal and elevated is not a sharp line but a transition.

The objection that a value with four different limits is of little use is justified. Nevertheless, something else is decisive: liver values are the cheapest and fastest indication that a closer look may be worthwhile, and in this role they are difficult to replace.

What matters when reading liver values

If you take away one single action from this article, let it be this: Take your latest test report and mark which liver values belong to the cell-damage group and which belong to the bile-flow group. Put GPT and GOT in one column, and gamma-GT and alkaline phosphatase in the other.

The reason is unremarkable. This classification shows within two minutes whether there is a pattern at all, and it is the question with which it makes sense to begin a conversation at the doctor’s office.

The starting question was which values matter. The most honest answer is: none on its own. What matters is which ones are elevated together.

The value becomes meaningful for the liver only in relation to alanine aminotransferase: In alcohol-related liver disease, this ratio is typically above 2, while in most other cases it is below 1.

Do I need to fast for liver tests?

The MSD Manual explicitly lists fasting as a requirement: Laboratory tests should be performed while fasting, because oral intake causes slight elevations (as of August 2025). If a value is well above the reference range, breakfast makes little difference; with a borderline elevation, it can determine whether the result appears above the threshold on the printout. Also worth noting: According to IQWiG, strenuous exercise can temporarily increase the values (as of March 20, 2025).

When should elevated liver values be evaluated by a doctor?

Immediately if you notice yellowing of the skin or eyes, pale stools, dark urine, persistent itching, pain in the upper right abdomen, fever, or unintentional weight loss. In the case of a mild, isolated elevation below twice the normal value, the MSD Manual initially recommends repeating the test, because the level normalizes in about one-third of cases (as of August 2025). If it remains elevated, a series of further tests follows—none of which can be represented by a self-test.
